On Christmas Eve in 2006—while I was carving a roast—he asked me to help him edit and revise Anatomy of the Foot and Ankle. Without hesitation or a second thought I accepted. We set forth to update the first 10 chapters (Part I) with perti- nent international literature from 1992 through 2008. Special attention was paid to Chapter 10 to emphasize the lamina pedis changes throughout the gait cycle.
Four additional chapters (Part II) were added relative to ap- plied anatomy. Normal arthroscopic anatomy, radiology, and angiosomes are emphasized and correlated.
We hope the third edition will aid the clinician—whether diagnostician, radiologist, regional anesthesiologist, or sur- geon—in the application of anatomic basics to the treatment of the foot and ankle. As Dr. Sarrafian has taught me well after his retirement, one needs to “water his mind with knowledge to allow it to grow.”




The first edition of Anatomy of the Foot and Ankle offered a new platform of knowledge. The warm and supportive reception given by my colleagues encouraged me to expand the horizons of this work, guided by the clinical and surgical necessities requir- ing “further clarifications.” The benchmark of the knowledge of anatomy is the cross-sectional anatomy. A complete, systematic, cross-sectional study of the distal leg-ankle-foot was done and added in Chapter IX under the title “Cross-Sectional and Topo- graphic Anatomy”. The methodology used to prepare the fresh- frozen sections was as described in the first edition.

The cross-sectional study made it possible to demonstrate the formation and the continuity of the compartments from the leg into the ankle- foot. This unified concept of the topographic anatomy is of prime importance for the clinical correlations. Clarification was provided to the concept of compartments and spaces in the foot as exemplified by the adductor compartment and adductor space. In lieu of surgical approaches, surgically oriented, topo- graphic dissections were conducted, bringing forth the pertinent anatomical relationships and possible pitfalls.

Anatomy,” was completely revised with the goal of providing a unified source for the enormous contributions made in the field of functional anatomy from the early 1980s to the pres- ent. A great effort was made to integrate the results of the excel- lent investigative works of Van Langelaan and Lundberg and colleagues using the innovative x-ray photogrammetric meth- odology. The foot-ankle is and should be considered as a functional unit. The synarthrodial concept of MacConaill, integrating the ligamentous function with the osseo-geometric anatomy, is of prime importance. In the biomechanical understanding of the foot-ankle unit, unless the ligaments are taken into consider- ation, the truth may forever remain evasive.
