greater than ever. These and other factors pose threats to patient safety, and these threats can be addressed through risk management.

Introduction  The ancient Hippocratic oath exhorts doctors to ‘first, do no harm’.  Unfortunately, it sometimes happens that patients suffer harm from healthcare that is intended to help them. As healthcare  becomes more complex, there is potential for the incidence of actual harm, as well as the incidence of ‘near-misses’, to increase substantially. One approach to preventing this is risk management.

Risk management systematically identifies factors that could
expose patients, staff, visitors and hospital property to harm and
puts in place barriers that could minimize the chance of harm
occurring. It empowers clinicians to use their skills and expertize in
maintaining a safe healthcare environment. Although it is impor-
tant that gynaecology departments, as organisational units, should
have structures and processes in place to manage risk, it is also
important that individual gynaecologists should incorporate the
principles of risk management into their clinical practice.
This paper describes risk management principles and tools
that can be applied at departmental and individual levels. The
review focuses on patient safety and does not directly cover
issues relating to the safety of staff, visitors or property. The
RADICAL risk management framework is described and applied
to gynaecological risk management, and common sources of
patient safety incidents in gynaecology clinics, wards and oper-
ating theatres are highlighted.
